In order to register for enrollment at Woodson ISD, students MUST meet the following requirements:

1. ONE OF THE FOLLOWING:

• Student lives within the Woodson Independent School District School Zone - Woodson School Zone Boundary Map

• Student has an approved transfer to Woodson ISD - Transfer Student Information

2. Student can be registered ONLY BY PARENT or LEGAL GUARDIAN**

• In case of divorce, parent must have custodial rights to register student.

• **Legal guardianship is established by a court.

• A notarized letter does NOT suffice as proof of legal guardianship.

3. Student must have ALL REQUIRED DOCUMENTS listed below:

• Government issued Photo identification of registering parent or legal guardian. (Drivers License, State I.D., Passport, Military I.D. with address matching the utility bill or lease)

• Student Birth Certificate

• Proof of Custody if applicable. (Most recent Divorce Decree concerning custody)

• Proof of Residence - One of the following(For those enrolling within the District School Zone)

  • Recent (within 3 months) utility bill (electric, water, gas) in the name of parent/guardian indicating services address matching the identification.

  • Copy of the lease listing parent/guardian as residents - address and phone number of leasing office MUST be provided (The address must match the parent/guardian identification)

• Immunization / Vaccination Record and current health insurance information.

• School Records from the previous school. (This includes an unofficial transcript reflecting all credits earned and the most recent report card, disciplinary reports, etc.)
